Adult Formation Enrollment
Welcome to St. Patrick Catholic Church Adult Formation Enrollment.

 This form is for enrollment of any adult formation classes i.e., Baptism Classes, Bible
 Studies, RCIA, etc...

 Complete the form and select the class or classes you would like to attend. If there is a
 cost for books, i.e., Bible study, you will be sent instructions on how to take care of that
 when you receive your enrollment confirmation letter. No one is ever denied the chance
 to heal, grow and practice their faith, because of financial difficulties. This concern
 should not be an influence in a person’s decision to enroll in a class. There is no fee for
 Baptism classes. Contact the parish office if you have questions regarding fees: (859)
 356-5151 or email: stpatsemail@fuse.net.

 Complete the registration for each person (you may add multiple students when you
 register). A confirmation statement will be sent to you via email with a payment coupon.
 Then send in your enrollment or textbook fee with the coupon via one of the following
 methods:

 - Mail to St. Patrick Church at 3285 Mills Rd., Taylor Mill, Ky 41015

 - Place it in the dropbox located by the secretary's office door, which across from the
 Adoration Chapel entrance 24/7.

 - You may also drop it off in person during the secretary's office hours:
  6-9 pm Mon-Fri (except Thu) & 12-4pm on Sat

 - Bring it with you when you come to class.
